Have You Misplaced or Lost Your COVID Vaccination Card?

In all the rush and craziness to make sure you got your COVID vaccine, you may now be realizing that you have misplaced or lost your vaccination card. If that is the case, you can now go on to a statewide portal that will allow you to print out a PDF of your own personal vaccine information. All you will need to do is to sign in to the COVID vaccine portal by creating a username and password, if you do not already have one. If by chance you have forgotten your username and/or password, please visit the following tutorial for instructions on how to retrieve this information.

Once in the portal, you will find an area at the top of the page entitled, “Generate Vaccine Information PDF.” Once there, you will have access to your COVID vaccination information, which will allow you to print this out for your own personal use.    

If you experience issues getting into your COVID-19 Vaccine Portal account, please contact the COVID-19 Vaccine Management System (CVMS) Help Desk Portal at https://ncgov.servicenowservices.com/csm_vaccine or call the COVID-19 Vaccine Help Center at (888) 675-4567 during the hours below: 

  • Weekdays: 7AM – 7PM 
  • Weekends: 8AM – 4PM

If by chance you have lost or misplaced your COVID vaccination card; but you do not have access to a computer, please feel free to contact Rowan County Public Health at 980-432-1800 or contact the agency that you got your vaccination from. There should be staff onsite that can assist you.

In closing, if you have still not received your COVID Vaccine; but you  are interested in doing so, please visit the CDC’s vaccine finder. Here, you will find a number of local agencies/businesses that are offering COVID vaccines in Rowan County, such as:

  1. Novant Health at the J. F. Hurley YMCA; Walk-ins are accepted M-F 8 am – 5 pm 
  2. Larger chain pharmacies: CVS and Walgreens
  3. Grocery Stores: Walmart, Food Lion (Salisbury and China Grove), Sam’s Club
  4. Local pharmacies: Moose Pharmacy, The Medicine Shoppe, Country Pharmacy, Price Pharmacy, and Cannon Pharmacy
  5. Provider offices: Rowan Diagnostic Clinic, Cleveland Wellness Clinic

To find out more information about which vaccines are being offered and when they are being offered, please contact the agency/business directly. Let’s make it a local goal to get as many people vaccinated in Rowan County as we can so we can enjoy summer and all the festivities that go along with it in a safe and healthy way.
